Use this form to show that there is a reasonable basis for believing that the purchase suits your current investment goals and financial situation, and that you have been reasonably informed of the terms and features of a variable annuity and would benefit from certain features of a variable annuity.

 

Anti-Money Laundering Training

All Insurance Producers must complete Anti-Money Laundering (AML) training before soliciting an annuity application. If you have taken AML training, please send a copy of your certificate from the training provider to:
